Документи, які необхідні для укладення Договору оренди біржового місця і надання права на участь в торгах (Documents necessary for conclusion of Exchange Place Lease Agreement and provision of the right of participation in trades)

Documents shall be provided in the original language and translated into Ukrainian.

1. Легалізований витяг з торгового, банківського чи судового реєстру, виданий не пізніше, ніж за місяць до дати подання документів для акредитації, або нотаріально засвідчене реєстраційне посвідчення уповноваженого органу іноземної держави про реєстрацію відповідної юридичної особи/фізичної особи, що здійснює підприємницьку діяльність; A legalized extract from the trade, bank or court register, issued no later than one month before the date of submission of documents for accreditation, or a notarized registration certificate of an authorized authority of a foreign state on the registration of the relevant legal entity/natural person engaged in entrepreneurial activity;

2. Довідка з банку (інший документ за підписом уповноваженої особи з печаткою (за наявності) нерезидента), що містить інформацію про реквізити банку, в якому відкрито рахунок, та номер банківського рахунку; Certificate from the bank (another document signed by an authorized person with a seal (if available) of a non-resident) containing information about the details of the bank in which the account is opened and the bank account number;

3. Довідка за підписом уповноваженої особи з печаткою (за наявності) нерезидента (інший документ), що містить інформацію про структуру власності, кінцевих бенефіціарних власників (контролерів) та зміст (види) діяльності, видана не пізніше, ніж за місяць до дати подання документів для акредитації. Інформація про структуру власності, кінцевих бенефіціарних власників (контролерів) подається лише юридичними особами; A certificate signed by an authorized person with a seal (if available) of a non-resident (other document) containing information on the ownership structure, ultimate beneficial owners (controllers) and content (types) of activity, issued no later than one month before the date of submission of documents for accreditation. Information on the structure of ownership, ultimate beneficial owners (controllers) is submitted only by legal entities;

4. Трудовий договір (контракт, наказ, протокол або інший документ) про призначення керівника юридичної особи; An employment contract (contract, order, protocol or other document) on the appointment of the head of a legal entity;

5. Документ, що засвідчує особу фізичної особи, що здійснює підприємницьку діяльність/керівника юридичної особи; A document certifying the identity of a natural person engaged in entrepreneurial activity/head of a legal entity;

6. Документ, що засвідчує особу (осіб), що буде (будуть) виконувати повноваження брокера (брокерів); A document certifying the person (persons) who will (will) fulfill the powers of the broker (brokers);

7. Довіреність (Довіреності) на особу (осіб), що буде (будуть) виконувати повноваження брокера (брокерів), оформленої згідно із законами країни, де нерезидент офіційно зареєстрований; Power of Attorney (Powers of Attorney) for the person (persons) who will (will) perform the powers of the broker (brokers), drawn up in accordance with the laws of the country where the non-resident is officially registered;

8. Заява про приєднання до Регламенту. Application for joining the Regulations.

9. Заява про приєднання до публічного Договору. Application for joining the public contract.

10. Анкета юридичної особи (суб’єкта господарювання – резидента України / нерезидента України) на виконання вимог Закону України «Про запобігання та протидію легалізації (відмиванню) доходів, одержаних злочинним шляхом, фінансуванню тероризму та фінансуванню розповсюдження зброї масового знищення» Questionnaire of a legal entity (a business entity – a resident of Ukraine / a non-resident of Ukraine) in compliance with the requirements of the Law of Ukraine "On prevention and countermeasures against legalization (laundering) of proceeds obtained through crime, financing of terrorism and financing of the proliferation of weapons of mass destruction"

After the registration is completed, your e-mail address will receive a notification about the decision of the Supervisory Board to lease you an exchange place. In case of a positive decision, a package of documents is attached to the e-mail, which you print, sign, seal and in paper form together with the originals of the documents provided by you in electronic form, send to the Accreditation Department at: Ukraine, 36039, Poltava, Shevchenko 52 str., Ukrainian Universal Exchange LLC.
